WAIVER & AGREEMENT (PLEASE READ CAREFULLY)

As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate in the designated sports and agree to the entry fee of three MILO 400g pack per participating TEAM, in addition I am fully aware that for the aforementioned child to be eligible to participate in this activity they MUST be 12 years of age PRIOR to August 1st 2019 (Photo ID or a passport sized photo MUST be provided with entry and a Valid passport for travel if successful is required) I understand that there are certain risks of injury inherent in the practice and play of the aforementioned sports, and other related activities incidental to my child’s participation, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the designated sport and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as listed above. In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless the organization named above and its partners, officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport and the activities incidental thereto, whether the result of negligence or any other cause. Inclusive of the above I grant use of my child’s image and likeness in photo and video format across ALL possible mediums for promotional and advertising purposes by NESTLÉ TRINIDAD and its affiliates indefinitely and globally.

THE MILO® FC BARCELONA FOOTBALL SKILLS TOURNAMENT
  • In T&T, the MILO® Champion Cup’s predecessor was the MILO® FC Barcelona Football Skills Tournament.
  • In 2017 and 2018, T&T introduced this Tournament to contribute to MILO® and FCB’s 22 million youth goal.
  • Through this tournament, NESTLÉ TT impacted over 900 young girls and boys locally, who came out to compete for the grand prize of an all-expense paid trip to Barcelona, Spain.
  • In 2017 3 youngsters went to Barcelona and in 2018, 5.
  • There they joined other children, ages seven to 12, and had the opportunity not only to train with Barça coaches for 2 days, but to also tour Barcelona City and Camp Nou.

Youngsters – both girls and boys – in their foundation years, ages 7 to 12, hailing from the more than 40 countries in which MILO® operates.

MILO® will host trials in Trinidad and Tobago to determine a winner, beginning in Tobago:

May 19th Mount Pleasant Grounds, Tobago
June 2nd UWI SPEC Grounds in St. Augustine on Sunday
June 2nd Dependent on demand, the possibility exists to host another tournament in Trinidad on this (reserve) date
June 9th Marvin Lee Stadium aka Ball Park, Trinidad

Players must be 12 (or under) on July 31st. If a player turns 13 before August 1st, 2019, they are not eligible to enter as this activity specifically targets pre-teens.

No limit has been established as NESTLÉ TT wants to allow the most children possible to have a shot at Barcelona, to bring home the Cup for T&T.

Coach Damian Cooper, a USSF licensed coach with more than 25 years of coaching experience and founder of Cooper’s Coaching Academy as well as TTFF referees.

NESTLÉ MILO® Champions Cup Rules and Regulations

  • All participating teams MUST register a coach who will be responsible for the team on the day

  • All participants need to provide the below COMPLETED forms prior to the tournament
    o Registration form
    o Picture or photo copy of participant birth paper o Valid Trinidad & Tobago passport
    o Passport sized photo

  • All participating teams must wear their own uniform to compete in, a milo sticker will be added to each player uniform for the tournament- All participants must wear a white on branded t-shirts. A MILO bib will be provided.

  • All participating teams need to be at the venue AT LEAST 30 mins prior to the start of the tournament

  • ALL participants need to be between 7 years or older on May 5th, 2019 and 12 years or younger on August 1st 2019 to be eligible to participate

  • Each team must comprise of two females, one female MUST be on the pitch at all times and the other a substitute

  • Each team MUST comprise of a goalkeeper and 4 outfield players

  • Teams comprising of players residing in Trinidad CANNOT participate in Tobago qualifier and vice versa

  • Players CANNOT be registered on more than one team for qualifiers and finals

  • Teams CANNOT change players from the qualifiers to the finals, any team doing so will be disqualified

  • If the winning team of the tournament cannot provide valid passports for travel for 1 or more players OR they 1 player cannot travel for whatever reason they will have to forfeit the trip and the opportunity will pass onto the second place team and so forth

  • Teams CANNOT replace players if they are successful in winning the tournament to travel to China in addition to the coach

  • Nestle Trinidad and its affiliates will NOT assist in procuring passports or American visas for travel

  • Parents and guardians expenses will NOT be covered to travel with players if they choose to do so, it will be at their own expense

  • Match officials will be from the Trinidad & Tobago football federation, their decisions are final

  • Each team must submit 3 packs of MILO 400 gram to participate.

  • NESTLÉ MILO® is not liable for any damages or injuries that may occur.
